ReEntry Program

Reentry Services at Route 33 exist to improve the overall health and wellness of people impacted by the criminal-legal system.
This is achieved by creating individualized care plans with a focus on providing wraparound supports for our clients. Case management may include Employment Services, Navigation of the referral system to receive treatment for mental health and substance use disorders, Medical care navigation through community organizations, and more, based on the identified needs and goals of each person.


Participants may also be eligible to receive material support for barriers such as Transportation, Housing, Utilities, and other basic needs (food, clothing, etc.).
Services are provided out in the community, as well as inside 10 correctional facilities around the state of
Indiana. Reentry Services focus on the dignity of each person, while promoting self-sufficiency and fostering resilience.

Consultation Call

This initial call helps us understand your current situation. Identify immediate needs and determine the best next steps for support. During this call, a case manager will complete a brief assessment, discuss available resources, and explain eligibility for ongoing services. This service is ideal for individuals seeking ReEntry or social services support, and who need to do an assessment before entry into the program.

Case Management

Our ongoing case management services provide personalized, one-on-one support tailored to your goals and needs. Clients work with a case manager to develop an individualized care plan, connect to community resources, and receive ongoing guidance related to Housing, Employment, ReEntry, and Basic needs (clothing, food)

Services focus on Empowerment and Long-Term Stability.

 Support Services

 Support Services are designed for individuals impacted by legal who are transitioning back into the community. Support may include referrals, employment readiness, care coordination, transportation assistance, and connection to essential services.
